The best source i know of is
http://www.historiccountiestrust.co.uk/
doesnt (yet) have all counties. But is working towards it.
It would be nice to get a public dataset :)
As to changes, I dont think they changed that much - in recent times,
but over the years they have changed, the above site should have more
about this
