Always in Meters is the one true path. The VTP software and formats let you
use any horizontal CS in the world, but vertical is Always in Meters. If
the user want to see feet in their GUI, fine, but don't muddy storage with
such things.
As for what to do in the absence of elevation, that would be a rather
app-dependent decision.
